รายชื่อผู้มีสิทธิ์สอบวัดความรู้เรื่องความปลอดภัยทางเคมี ในวันจันทร์ที่ 30 เมษายน 2561, 16.30-17.30 น.
**สอบ ณ ห้อง 222 ชั้น 2 อาคารแถบ นีละนิธิ คณะวิทยาศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย**
ข้อปฏิบัติและรายละเอียดในการสอบ
1. โปรดนำบัตรประจำตัวนิสิต หรือบัตรข้าราชการ หรือบัตรพนักงานมหาวิทยาลัย เพื่อยืนยันตนระหว่างการสอบ
2. โปรดวางกระเป๋าและเอกสารการเรียนไว้บริเวณด้านหน้าห้องสอบ
3. ใช้เวลาสอบ 1 ชั่วโมงและไม่อนุญาตให้ออกจากห้องสอบก่อนหมดเวลาสอบ
4. ผู้ที่สอบผ่าน คือ ผู้ที่ได้คะแนนสอบอย่างน้อย 70%
5. ประกาศผลสอบในวันอังคารที่ 15 พฤษภาคม 2561 ผ่านทางเว็บไซด์ www.chemsafety.research.chula.ac.th
The chemical safety exam will be held on Moday 30 April 2018 (4.30-5.30 pm) at room#222, 2nd fl, Tab Niranithi building, Faculty of Science, Chulalongkorn University.
Remarks
1. Please bring your Chulalongkorn U. student or staff card to prove your identity during the exam.
2. All personal belongings (backpacks, book, bags, purses, etc.) as well as handouts need to be left in front of the exam room.
3. Due to security of your belongings, you are not allowed to leave the exam room before 5.30 pm
4. To receive the certificate of participant, the examinee needs to have a score at least 70% to pass the exam.
5. The result of exam will be announced on www.chemsafety.research.chula.ac.th on 15 May 2018.

หมายเหตุ
1. ผู้เข้าอบรมครบทั้งหลักสูตร (อบรมครบทั้ง 3 วัน) จะมีสิทธิ์ในการสอบวัดความรู้ เรื่องความปลอดภัยทางเคมี ในวันจันทร์ที่ 30 เมษายน 2561, 16.30-17.30 น.
2. ผู้ที่มีรายชื่อแต่ไม่ได้รับอีเมล์แจ้งกลับเรื่องเอกสารการอบรมจาก “chemtrain” ให้ลองตรวจสอบอีเมล์ใน SPAM mail (Junk mail)) ถ้าพบว่ายังไม่มี ให้ดำเนินการได้ 2 วิธี ดังนี้
วิธีที่ 1 ขอไฟล์เอกสารการอบรมผ่าน chemsafe_training@hotmail.com หรือ
วิธีที่ 2 ดำเนินการถ่ายเอกสารที่ร้านถ่ายเอกสาร (ข้างร้านกาแฟอินทนิล)ใต้อาคารมหามกุฏ คณะวิทยาศาสตร์ (ต้นฉบับเอกสารการอบรมความปลอดภัยทางเคมี (ฉบับภาษาไทย)ได้ถูกฝากไว้กับทางร้านเป็นที่เรียบร้อย)
3. ตรวจสอบความถูกต้องของ ชื่อ-นามสกุล ในกรณีที่พบว่าไม่ถูกต้อง โปรดแจ้งข้อมูลที่ถูกต้องกลับผ่านทางอีเมลล์ chemsafe_training@hotmail.com
4. ขอสงวนสิทธิ์การเข้าอบรมเฉพาะนิสิตและบุคลากรของจุฬาฯ เท่านั้น

2. สามารถรับใบสำคัญการฝึกอบรมความปลอดภัยทางเคมีได้ดังรายละเอียด:
สถานที่: ห้อง 804/1 ชั้น 8 อาคารวิจัยจุฬาลงกรณ์มหาวิทยาลัย (อาคารอยู่ด้านข้างอาคารวิทยาลัยปิโตรเลียมและปิโตรเคมี) โดยติดต่อขอรับใบรับรองได้ที่ คุณ นิภาพร หรือ คุณจอห์น
เวลา: สามารถติดต่อขอรับใบรับรองได้ในทุกๆ วันพฤหัสบดี ช่วงเวลา 13.30-16.00 น ตั้งแต่วันพฤหัสบดีที่ 3 พฤษภาคม – 7 มิถุนายน 2561 (ในกรณีที่มีความจำเป็น ที่ไม่สามารถขอรับใบรับรองในช่วงวันดังกล่าว สามารถขอรับใบรับรองในช่วงวันและเวลาอื่น ภายในเวลาราชการได้ โดยติดต่อเพื่อทำการนัดหมายก่อนที่ คุณนิภาพร หรือ คุณจอห์น โทร 02 218 3962 หรือ ที่ ดร.องอาจ โทร 083 547 5991)
3. โปรดแสดงบัตรนิสิต หรือบัตรข้าราชการ หรือบัตรพนักงานของมหาวิทยาลัยเพื่อแสดงตนในการรับใบรับรอง
กรณีที่สอบไม่ผ่านในครั้งแรก หรือมีสิทธิ์สอบแต่ไม่ได้เข้าสอบ*
1. สามารถยื่นความจำนงสำหรับการสอบครั้งถัดไป (โดยไม่จำเป็นต้องเข้าอบรมใหม่) โดยส่งข้อมูล ชื่อ-นามสกุล ภาควิชา และคณะหรือหน่วยงาน มาที่อีเมลล์ chemsafe_training@hotmail.com ตั้งแต่วันนี้จนถึงวันจันทร์ที่ 25 เมษายน 2561 และ สามารถตรวจสอบรายชื่อของผู้มีสิทธิ์ในการสอบความปลอดภัยทางเคมีได้ในวันเสาร์ที่ 28 เมษายน 2561 ผ่านทาง www.chemsafety.research.chula.ac.th
2. การสอบวัดความรู้เรื่องความปลอดภัยทางเคมีในครั้งถัดไป จะจัดสอบในวันจันทร์ที่ 30 เมษายน 2561 เวลา 16.30-17.30 น. ณ ห้อง 222 ชั้น 2 อาคารแถบ นีละนิธิ คณะวิทยาศาสตร์
